Fix programming to include the Course Approval Code in the user's enrollment record when using Invite/Register/Cancel

E-sign - Hold course from Invite/Register/Cancel menu until the Superintendent Designee signs

E-sign - Hold course from Course Registration menu until Superintendent Designee signs
