📝 Create New/View Eligibility Forms V2

Creating an Eligibility Form determines if the student has a mental or physical impairment that substantially limits a significant life activity(s), which meets or does not meet Section 504 eligibility criteria.

  1. Locate Eligibility Forms in the left navigation menu and choose Create a 504 Plan Eligibility Form.

  1. In the Parties section,  select the Student (Required) and 504 Coordinator (Required).
  2. In the Eligibility Criteria and Determination section, choose one of the three statements to indicate whether the student meets the eligibility requirements.

  1. In the Area of Disability section,  fill in the Area of Disability (Required).
  2. In the Accommodation Types section, select the Accommodation Types: Instructional, Environmental/Accessibility, Assessment/Testing, Behavioral/Social, Other.

  1. In the Important Dates section,  the Creation Date (Required) will be prefilled with the current creation date. Filling in the Date Notice Sent, Date Consent Received, and Date of Meeting is Optional.
  2. In the Notes section,  explain the Nature of Concern (Required), the Impairment Description (Required), the Impairment Limitation Description (Required), and other Notes (Required)

  1. Click Save.
  2. Use the Action Gear button in the right corner of the page to Print or Convert to 504 Plan.

  • Selecting Convert to 504 Plan will prepopulate additional fields for completion:
    • Student Name
    • 504 Coordinator Name
    • Area of Disability
    • Implementation Date (date of creation) 
    • Review Date (1 yr from date of creation)
    • Periodic Re-Evaluation Date  (3 yr from date of creation)

View 504 Plan Eligibility Form

  1. To see all eligibility forms, clear the filter in the My Eligibility Form column.

  1. Users may select multiple records to print by clicking the Action Gear in the top-right corner, then selecting Print Selected Prior Eligibility Form from the drop-down menu.

  1. Users may also use the row actions gear to the far right to Edit or Delete.
