• [APP504-79] - On the Eligibility Form, under the notes section, all four sections of notes for: nature of concern, impairment description, impairment limitation description, and notes, are now displaying on the printed form.

• [APP504-84] - The Parent Signature and Consent Section has now been added to the printed Eligibility Form.
