Add SSID and OtherID to User defined assessment template

Many educators are adding results from third-party assessment sites to Homeroom via the user-defined assessment tool for entire schools. They use copy/paste for the scores but need to be sure they are pasting into the correct student record and do not use our StudentID. Please add two additional columns to identify students, the SSID and the OtherID to ensure accurate score results.

  In the Sept 30, 2016 Homeroom release, the Other ID and SSID numbers were added to the download template for user-defined assessments.
